Job Description

Registered Dietitian - Homecare/Infusion

Status: Casual/Per Diem

 

Maintains an environment of safety for patients, self and others.

Takes and evaluates all new referrals for insurance qualification criteria. Coordinates with Reimbursement for CMN requirements.

Develops and updates the plan of care based on assessment of the patient's nutritional needs, physician's orders and assessment of the capability of the patient/caregiver to understand the patient's nutritional goals.

Evaluates TPN patients using reimbursement guidelines to ensure payment of services.

Performs all equipment teaches prior to patient's hospital discharge date.

 

Instructs patients as needed using appropriate teaching materials for diet instructions, with no issues reported. Adapts instruction to meet patient's needs i.e. education level or cultural diversity.

Accurately calculates and recommends enteral and parenteral nutrition.

Develops and updates the plan of care based on assessment of the patient's nutritional needs, physician's orders and assessment of the capability of the patient/caregiver to understand the patient's nutritional goals.

Assist staff involved in the patient's care, the patient and caregiver in understanding and following recommendations related to the plan of care.

Observes and reports to the attending physician and other staff involved in the patient's care, the patient's reactions to and tolerance for the nutritional therapy and any changes in the patient's physical status or in the plan of care.

Evaluates all new TPN therapy referrals and high risk patients for clinical safety, appropriateness and nutritional adequacy.

Performs home visits, as needed. Documents visit and assessment with education parameters and interventions.

Follows through to provide continuity of care and reassesses patients, as appropriate.

Successfully manages an established caseload of enteral patients.

Maintains reports (number of referrals, referral sources, patients on service, discharges, hospitalizations) and collects data on caseload to ensure quality services.

Works with warehouse staff to ensure adequate levels of formula, based on estimated needs, availability of enteral supplies and equipment.

Coordinates the delivery of orders with the Customer Service Specialist to ensure billing dates are correct and any formula/supply changes are updated on delivery ticket.

Informs management of potential issues, trends and opportunities for improvement.

Obtains all order renewals, as needed, to maintain active status and reflect current medical nutrition therapy regiment.

Progress notes are written appropriately and timely.

Updates policies and procedures and patient teaching materials.

Nutrition care plans include age-specific information as appropriate to patient needs, without exception.

Serves as a resource for staff, case managers and referral sources.

Participates in education and marketing opportunities for case managers, referral sources, etc.

Provides in-services to staff and outside sources, as requested. 


Assists with orientation of new co-workers, as needed.

Attends continuing education seminars and classes, as needed.

Provides support for enteral equipment issues, malfunctions, re-teaches, and pump maintenance pick-up and delivery.

Works with marketing staff to identify potential growth opportunities for home enteral and parenteral nutrition therapies. Develops and maintains nutrition customer relationships. Works with marketing staff as the nutrition support content expert.

Is available on-call after regular business hours to act as the nutrition expert for internal and external sources and patients.

Coordinates any delivery needs for enteral patients that arise during on-call hours.



Qualifications

Experience: One to three years prior experience as Clinical Dietitian with one year in HomeCare desired.


Education: Bachelor's degree (B. S.) in Nutrition and Science or related field preferred. Completion of dietetic internship.

 

Licensure: NC Driver's License, LDN and RD required
